Throw the card from below the first form upwards so that the second form spawns in the path of the card. I’ve played about 300 of Isaac Rebirth overall, though with very long breaks. This is the simplest solution, especially since you can destroy both forms of the boss with one card. I’ve played about 300 of Isaac Rebirth overall, though with very long breaks.
